ABOUT THE ROLE
Therapeutic Case Management Responsibilities
1. Provide therapy and case management for a maximum of 6 residents at a time with the following responsibilities:
a. Develop individual care/treatment plan and conduct continuous assessment, revision, and implementation of treatment plan for the resident and her family to achieve therapeutic goals
b. Plan, implement and provide supplementary interventions relevant to the resident’s overall treatment plan
c. Provide counselling and support to the resident in individual sessions as part of the treatment plan
d. Coordinate and manage timely completion of Trauma System Therapy (TST) forms, reports and progress of treatment with the Care Team
e. Provide interventions and support in family work by using the TST HELPers Guide and arranging for family sessions as part of the treatment plan
f. Maintain detailed documentation and reports on each resident’s progress
g. Provide updates and reports to the interdisciplinary team to facilitate decision making on treatment plans for the residents
h. Provide regular updates and management of resident’s data on the online MSF ECRM system including application of the Medical Exemption Fee card (MFEC) for residents
i. Liaise with the MSF case worker and external agencies to provide coordinated care for the residents
j. Liaise and work with schools to facilitate and support resident’s reintegration back to school
k. Recommend external therapies relevant to resident’s overall treatment plan
l. Provide linkage to community resources for residents and their families, to facilitate the reintegration of these residents into the community
2. Participate in planning and running clinical groups focused on building emotional regulations skills, building life skills, and trauma psychoeducation
Clinical/Therapeutic Responsibilities
1. Plan and run groups with residents focused on:
a. Building emotion regulation skills
b. Building life skills
c. Regulating the Brain
d. Psychoeducation on trauma
2. Take on the Champion role in charge of the training and implementation for clinical Residential Management Support (RMS) tools, such as:
a. Consumer Orientation
b. Regulate My Brain
3. Provide training and boosters to direct care staff as instructed by supervisors.
Meetings
1. Coordinate and facilitate monthly Care Team meetings including:
2. Collecting updates from Psychologist and Coach about the resident’s residential and treatment progress
3. Working with the Coach in the implementation of Care Team decisions regarding behaviour management, H&I interventions, and therapeutic direction
4. Completion of Child Ecology Check-In (CECI) with the team
5. Contribute to interdisciplinary case review meetings
6. Attend HCSA, Clinical and staff meetings
7. Attend MSF Case Review meeting for cases under her management
8. Attend meetings with schools and any external agencies pertaining to management of the cases
9. Participate in relevant Communities of Practice to further develop and share skills and knowledge with members of other communities of practice
10. Participate in special projects/programmes and committees.
Report/Paper Works
1. Complete the Counselling & Family sessions notes and record in MSF monthly Muster
2. Ensure submission of incident reports of cases under her management
3. Submit other relevant reports required by Management, MSF or other agencies
Others
1. Perform additional duties as required by the nature of the position or as requested by Head of Home
CANDIDATE REQUIREMENTS
Education And Experience
1. Minimum degree (preferably in social work or psychology or equivalent qualification) with 1-2 years relevant work experience
2. Knowledge and understanding of children/young persons with high level of needs
3. Understanding of trauma, attachment and resilience theory, and child and adolescent development
4. Possess professional documentation and charting skills
5. Have experience working with youth-at-risk
6. Possesses working knowledge of Trauma Theory and Trauma Systems Therapy
7. Trained in working with abuse and emotional dysregulation
8. Trained in CANS, YSL, and SDM-PSF
9. Trained in CPI Safety Interventions - Foundation and Advanced & Emergency
Other Requirements
1. Trained in Trauma Systems Therapy (TST)
2. Maintain a certification in Child and Adolescent Needs and Strengths (CANS)
3. Maintain a certification in CPR and First Aid
