Support in implementing Dementia Singapore Ltd (DSG) palliative care initiatives. Develop a palliative care knowledge management framework that enables DSG to embrace palliative care principles and practices across the care services, including the community care sector at large.
Responsibilities:
- Provide knowledge management support to the Team Lead in the development of a palliative care framework.
- Collaborate with team members and external parties in the development and consolidation of palliative care resources.
- Oversee the development of palliative care materials and resources to advance palliative care practices in the community care sector.
- Ensure information is well presented and easily accessible via different channels and media.
- Maintain and update palliative care information and resources.
- Encourage a knowledge-sharing culture across Dementia Singapore.
- Provide secretariat support to the team lead in organising meetings, i.e., Advisory Committee.
- Liaise with the corporate communication department and funder in publication and publicity-related activities.
- Participate in team meetings and case conferences as required.
- Provide statistics and reports for programme evaluation and reporting purposes.
- Any other duties as assigned.
Requirements:
- Degree in Health Sciences or related field.
- Experience in the community care sector and passionate about dementia care
- Familiar with the person-centred approach to dementia care.
- Able to publish journal articles.
- Excellent oral, written and communication skills.
- Able to present information in a creative and succinct manner.
- A good team player.
- Fast learner and the ability to synthesise knowledge and new information.
