Reporting to the Resident Physician, this role functions primarily as the case manager for SingHealth Community Hospitals (SCH) inpatients referred into SCH’s post-discharge programmes (e.g. Integrated Primary Care for At-Risk Elders [iPCARE]). The case manager facilitates the development of joint care plans by working with internal and external stakeholders (families, nursing homes, community care providers, general practitioners, etc.) to ensure a smooth transition of patients back to the community or institutional care setting. This includes the coordination of patients’ and families’ clinical and psychosocial care needs and management of referrals.


The incumbent will also assist patients, families, caregivers and partners such as nursing homes and general practitioners in understanding patients’ conditions, and monitor continued care plans with these respective parties. As these post-discharge community programmes are refined over time to enhance care models according to the operating environment, the case manager, as a key care team member, will participate actively in the evaluation of programmes and related quality improvement projects.


This role will also require travelling to patients’ homes, general practitioners’ clinics and/or nursing homes for coordination or ongoing monitoring of patients' conditions.


Job Requirements

  • Minimum Diploma in Nursing
  • Valid Practising Certificate with Singapore Nursing Board (SNB)
  • At least 2 year(s) of relevant working experience. Previous exposure in eldercare and other community care settings will be an added advantage
  • Familiar with Microsoft Office applications
  • Able to communicate effectively with elderly patients.
  • Passionate with a positive and caring attitude towards patients, internal and external clients
  • Able to work independently as well as in a team
  • Good communication and coordination skills
  • Adaptable across different care settings

Working hours: 8.30 am to 6 pm (Monday to Friday/ no shift work)
