Responsibilities:
- Responsible for all admission and discharge procedures of residents
- Maintain records on the progress of the resident/ referral
- Evaluate referrals. Conduct means test, case management and liaison with residents and their family members. Assess and recommend fees payable
- Undertake home visits and other interventions (family conference, case conference, project care) to help residents and families
- Work in partnership with residents and their families to assess and address needs to enhance the quality of life, independence, and social integration
- Prepare weekly report, monthly report, and other reports for Management
Requirements:
- Degree/ Graduate Diploma in Social Work
- At least 3 years of relevant working experience, preferably in Social Service Sector
- Effective Report Writing Skills
