Role Summary:
Case Managers are instrumental in supporting our work of short term financial support to families and individuals who have fallen on hard times. You will be required to engage with beneficiaries and community partners through collaborative approaches in order to mobilise resources and support for those who need it.
Key Responsibilities:
1. Client Intake Process
· Assist in the intake process for new clients, gathering basic background information and ensuring clients are aware of ROH’s Campaign model (Beneficiary Kit).
· Collate required forms and documentation.
2. Client Assessment and Support
· Conduct client interviews to identify needs, strengths and challenges.
· Communicate appointment details and reminders to clients as needed.
· Complete case assessments and make recommendations for beneficiary campaigns.
· Write campaign stories to aid with beneficiary fundraising.
3. Case Management and Resource Coordination:
· Follow up with beneficiaries in cases where there is a collaborative element in the Campaign. (e.g. sourcing for quotations, sourcing for items; seeking suitable employment)
· Contacting clients for updates in the process of campaigns.
· Collaborate with ROH team members on thematic Campaigns and Community Projects.
· Collaborate with social service professionals from other agencies in support of case goals.
· Monitor and document client progress.
· Refer beneficiaries to other community partners and social service agencies where applicable.
4. Administration and Reporting:
· Ensure accuracy and timeliness of data in systems and databases.
· Putting up of Campaigns on ROH website.
· Monitoring of disbursement schedule.
· Maintain accurate and up-to-date client records, ensuring confidentiality and compliance with relevant regulations(application forms, tracking sheet and updating list).
· Put in client updates for ongoing and completed campaigns.
5. Other duties as assigned by Reporting Officer
· This role requires you to support other team members in other ROH projects and events.
This might take the form of (and is not limited to):
o Event set up and support
o Facilitation of meetings
o Rotational admin support of key meetings
