Key Responsibilities
- Provide daily escort duties for clients from their home to the hospital or polyclinic for their medical appointments.
- Accompany the clients on the vehicle and keep an eye on them during the journey.
- Assist client with walking aids or on wheelchairs with their ambulation.
- Assist the clients throughout their hospital or polyclinic visit, while ensuring their safety at all times.
- Escorts/transports clients safely, professionally, courteously and in a timely manner throughout the hospital or polyclinic.
- Assist clients in boarding and alighting from the vehicle in line with clients’ needs; where necessary, via operation of wheelchair hydraulic lift or other mobility devices in line with proper, safe lifting techniques.
- Administer safety procedures for clients before and during transportation to ensure safe and secure vehicle environment. Ensure that clients/wheelchairs are safely secured in the vehicle.
- Escort the right senior to the right medical institution/clinic and room(s) for their medical treatment/consultation.
- Perform with medical appointment process such as clinic registration, settling of payments, future appointments scheduling, walk-in and collection of medication.
- Promote safety and comfort for seniors by interacting with them while waiting for transport during appointments.
- Liaise with office with client’s follow up medical appointment and transportation.
- Attend to senior’s needs and provide physical assistance such as buying meals, toileting, perform translation if needed) during appointments (i.e. on wheelchair)
- Hand over all medications, doctor’s notes, appointment schedules to the elderly or cluster support staff or social worker upon return to the Home.
- Inform the supervisor/manager of any abnormality of the client.
- Physically able to handle elderly on a wheelchair.
- Adhere to the Standard Operating Procedures (SOP) at all times.
- Exhibits a professional, friendliness, courteous, empathetic, compassionate and positive demeanor at all times to the clients.
- Comply with Personal Data Protection Act (PDPA) to ensure patient confidentiality, all sensitive information and privacy are safe-guarded.
- Assist in Meals-On-Wheels delivery as required.
- Perform any other duties or to undertake other responsibilities, etc as and when assigned by the supervisor or manager.
