This role is responsible for the care management of elderly clients under the maintenance day care programme, including assessment, planning, implementation and evaluation of nursing care as well
as client education.
The main responsibilities include:
• Take charge of the whole clinical nursing practice in the centre and ensure compliance with approved standards of nursing practices, including the creation and maintenance of policies
and SOPs.
• Perform all general nursing duties to provide quality nursing care and support to clients to
ensure their safety and comfort. Duties include medication administration, wound management, fall risks and hospitalization procedures, medical emergency procedures, infectious diseases outbreak procedures and other nursing duties as required.
• Conduct assessments of clients during admissions and periodic reviews, including preassessment of referrals from AIC
• Assess and evaluate client’s conditions to plan and implement individual care plan accordingly. Consult and coordinate with OTs and PTs and other healthcare professionals as necessary.
• Monitor, assess and review the progress of clients.
• Identify early signs and/or deterioration of clients’ condition through observation and
appropriate assessment tools.
• Ensure clients’ records are created, maintained and updated accurately on a timely basis
• Responsible for the administration and submission of documentation to relevant
government bodies
• Teach clients, caregivers and family members proper techniques in self-care
• Assist and support delivery of health promotion activities in the centre
• Participate in training and supervision of support care staff.
• Coordinate with programme staff to organize and participate in the centre programmes and
activities
• Other duties and tasks as assigned by the Centre Manager
Job Requirements:
• Diploma/Degree in Nursing
• Registered with Singapore Nursing Board
• Minimum 2-3 years of nursing experience
