The Job:
Your job responsibilities will include but not be limited to the following:
- Coordinate health and social care by liaising with various stakeholders within the integrated community care team
- Screen residents to determine their health and social care needs
- Coach patients and caregivers in managing patients' health and social aspects, including managing chronic diseases, adopting healthier diets, and sharing information on suitable community resources and services for residents
- Implement changes to care plans co-developed with residents, in consultation with Senior WBCs and/ or clinical teams
- Guide SAWBCs & AWBCs in care coordination & case management duties, patient coaching duties as well as health and social care needs assessments
Job Requirements:
- Bachelor Degree in any discipline, preferably in healthcare, nursing or sociology discipline
- Operational experience in and knowledge of health social and community care sectors preferred
- Good verbal and written communication skills
- Versatile to adopt a variety of coaching methods, as suited for different resident and patient profiles
