The Role: Case Worker
As a Case Worker in our Family and Life Services (FLS) Department, you will play a pivotal role in working closely with our students, clients, families, and allied professionals. Your primary focus will be on case management, ensuring the educational development of our students in Eden Children Centre and Eden School and supporting their ongoing learning in Eden Centre for Adults.
Key Responsibilities:
Engagement and Assessment
· Assess referral cases to determine their suitability for intake.
· Identify and clarify key issues in problem situations, conducting needs and intake assessments to assess severity, key risks, and protective factors for intervention.
Case Review and Documentation
· Objectively evaluate processes and outcomes, recommending possible improvements.
Goal Setting and Intervention Plans
· Collaborate with clients to develop care/case plans and set goals based on standard guidelines and professional assessments.
Programme Development and Implementation
· Assist in developing and implementing programs through data collection and basic analysis.
Involvement in Alumni and Respite Programs for AA(S)
Requirements:
· Bachelor’s degree in social work, Psychology, or Counselling.
· Relevant work experience in Case Management/Social Work in the Special Needs sector is a plus.
· Strong communication skills and comfort working within a multi-disciplinary team.
· Candidates currently pursuing training in Social Work/Counseling will be considered for the role.
