Responsibilities and Duties:
Specific
· Process and manage referrals for ACP discussions from inpatient, outpatient and community settings of SLH
· Facilitate ACP discussions with patients, families and nominated spokespersons
· Document ACP discussions at the appropriate platforms
· Coordinate between patients, families and healthcare team for ACP facilitation and follow up
· Make referrals to community services related to ACP matters
General
· Work with ACP Coordinator to provide support in ACP referrals and enrolment and data input as needed
· Conduct talks and workshops for healthcare professionals to raise awareness about and advocate for Advance Care Planning
· Participate in public awareness events (within SLH or jointly with partners) to raise awareness about Advance Care Planning
· Participate and assist in conducting audits, research, surveys or quality improvement projects related to ACP
· Preparation of reports and statistics as required.
Job Requirements:
· Professional qualification in Nursing, Social Work, Allied Health or/and any clinical disciplines relevant to the job.
· At least 3 years of experience as an Allied Health Professional
· Experience in a Hospital setting preferred
· Certification in Advance Care Planning Facilitation (General and Preferred Plan of care) and experience in conducting ACP discussions would be an asset
