Through our Active Ageing Centres, we engage the seniors within the clusters and support them by reaching out to understand their needs, curating programmes that cater to the needs of active seniors, and befriending isolated and vulnerable seniors.
As a Programme Associate, you play an important role in sharing our goal in the work of the AAC and assisting the Centre Manager in fulfilling the objectives of the AAC and meeting the targets set by the management and funders. You shall also look out for vulnerable seniors and support their needs.
Duties are as follows:
* Promote Health Active Ageing Centres' eldercare services and programmes.
* Conduct/support active ageing programmes and encourage active participation with seniors. The services include: conducting community screening assessments, organising and conducting active ageing programmes, provide befriending services (conducting home visits) and monitoring their vital signs.
* Where required, coordinate care and support services through internal referral or to external agencies.
* Provide support to seniors at outreach events and programmes.
* Carry out administrative functions such as data entry, record keeping and report submissions in a prompt and accurate manner.
Qualifications
* Minimum GCE 'A' level or Diploma.
* Proficient in Google Suite Applications.
* Able to work independently and at the same time work well as a team player.
* Genuine interest to engage our seniors in the community.
* Possess good interpersonal and communication skills.
* Pleasant, proactive and highly accountable for the work assigned
* Good knowledge of various eldercare services and support in the community setting.
* Ability to converse in local languages.
Working hour:
Monday to Friday, 8.30am to 6pm. You need to be open to working some weekends, and weekday nights for weekends, compensated with time off.
