Job Description:
· Conduct casework.
· Perform stakeholder engagement and partnerships.
· Perform community development work.
· Design, implement and conduct Groupwork and Interest Based Groups for elderly.
· Provide social and emotional support through home visits, care planning and activity recommendation.
· Provide support for volunteer management.
· Maintain and document records of groupwork and case sessions.
· Provide support to Case Managers.
· Coordinate, plan, develop, implement and evaluate programmes.
· Conduct environmental scans and community need assessments.
Job Requirement:
· Diploma in Social Work
· Experience in eldercare preferred
