Responsibilities
- Conduct pre-admission financial counselling to next-of-kin (NOK) or patients who are scheduled for admission for Nursing Home Long Term or Respite Care.
- Support Medical Social Workers in social work functions and financial matters for patients in the Intermediate Long-Term Care sector (ILTC) such asILTC schemes
Means Test
Medical Fee Exemption Card (MFEC)
Any other financial assistance
- Meet and liaise with patients / next of kin (NOK) for documentation and approvals in compliance to application processes.
- Assist in the management of cases that qualify for Medifund and the reporting to the Medifund Committee.
- Ensure that Advance Care Planning records in the national portal are updated.
- Maintain data records and perform other administrative tasks within the Case & Social Work Department.
- Undertake any other tasks as assigned.
Requirements
- Relevant 3 years of working experience is essential.
- Experience in Healthcare or Social Work industry will be an added advantage.
- Passionate about serving elders.
- Resourceful, proactive, responsible and team player with strong interpersonal, communication skills and patient-oriented personality.
- Ability to prioritise & complete tasks within deadlines.
- Mature and confident in dealing with patients, NOK and the public.
- Proficient in use of Microsoft Office applications.
