JOB SUMMARY
- To increase mental health literacy in the community through public education on mental health and psychosocial needs of people affected by mental illness and their families.
- To promote early recognition of signs and symptoms and facilitate early treatment for people-at-risk through enquiry service and public education.
- To facilitate navigation of Community and Mental Health services and to link people affected by mental health issues and their family members to relevant services.
- To provide basic emotional support to people affected by mental health issues and their family members.
KEY DUTIES AND RESPONSIBILITIES
- Public education and community outreach: Organise, coordinate and set up education and community outreach events
- Care coordination service: Provide care coordination (assessment, emotional support and basic counselling, psycho-education, referral, follow-up and organise case conference, etc)
- Referral sources and community partners: Network and engage partnership with community agencies; manage relations with referral sources and community partners
- Administration: Maintain case files and ensure all records, contacts with client, care plan and outcome of clients are properly maintained; submit statistics regularly; plan, obtain, collate and analyse feedback from clients/ family members
- Any other related duties as assigned by supervisor
