Job Summary:
To facilitate clients reintegrating back to the community within 2 years from admissions by providing:
a. Case management
b. Supportive counselling
c. Basic vocational support
d. Individual and family psychoeducation and support group
e. Engaging family, other stakeholders and natural supports in the reintegration process
Roles and Responsibilities:
1. Assess suitability of referrals seeking admissions
2. Provide care coordination and case management for up to 35 clients
3. Conduct psycho-education sessions
4. Provide basic educational and vocational support including imparting basic educational and vocational skills to support clients in their respective endeavours
5. Network and work with referral source, caregivers, community agencies and partners, relevant government agencies, and other stakeholders to provide holistic care and support
6. Conduct review on progress of each client within prescribed time-frame
7. Transition and discharge planning
8. Maintaining case files
9. Organising of social and leisure activities, festive celebrations, and other relevant events
10. Capture, collate and analyse relevant data on a timely manner for service review, statistical reports, etc.
11. Make home visits, accompany clients for medical review and undertake other out-of-centre duties as necessary
12. Coverage for colleagues away from work
13. Assist in other duties and functions as assigned by supervisor
Candidate Requirements
- Degree in Psycholoqy, Counselling, Social Work or related discipline
- 2 to 3 years of relevant experience
- Have a passion to work with persons with mental health issues
- Strong relational skills, independent and effectively bi-lingual (English and Mandarin).
- Ability to speak dialect an added advantage.
- Able to network and work with other stakeholders and community agencies
