GENERAL SUMMARY
The Social Work Associate provide support and guidance for residents in fulfilling the global objectives of programme. The Social Work Associate will be part of the casework and counselling team focusing on intake and referral, working with residents to build stability and self-reliance and work towards home ownership where possible.
ROLES & RESPONSBILITIES
Case Management Support
a. Intake – interview and allocate residents to suitable tooms based on their profile
b. Referral – link residents up with the appropriate services (government, community, private etc) to support global objectives of building stability and self-reliance and working towards home ownership where possible)
c. Engagement, assessment, intervention (referral) and evaluation of residents
d. Admission briefing and orientation
a. Case work or/and counselling support; Support case work, for example home/work visit, event and programme
e. Discharge planning and exit interviewsincluding preparing residents for eviction (subject to approval by HDB and initiated only as a last resort) and working with relevant social agencies/MSF for shelter accommodation for residents before eviction
2. Manage generic mailbox and prepare response to correspondence on routine inquiries, intake and referrals
a. Record keeping, documentation and data entry
Administration and Facilitation of programmes
b. Support planning, organizing and co-facilitation skills for groupwork and community work
c. Administrative support for the Clinical team
d. Support research endeavour
Audit
a. Support to monitor progress against relevant KPIs as required by funder and other stakeholders
b. Support in planning, implement and review processes to meet the achievement of KPIs and MSRs as required by funders
c. Ensure proper documentation is maintained, updated and reviewed on a regular basis for audit
Stakeholder relationships
a. Collaborate with other teams in fulfilling global objectives of the programme
b. Manage communications and feedback from stakeholders and team members
Build collaborative working relationships with partnering agencies
Others
a. Any other duties assigned by supervisor from time to time
QUALIFICATIONS
Education And Experience
1. Minimum certification level qualification in social work, counseling or psychology
2. Digitally fluent; familiar with digital tools for basic data visualization, basic data analysis, report-writing and presentation
3. Knowledge of relevant social issues such as homelessness, poverty preferred
