- Coordinate and Schedule Diabetic Foot Screening Services, Diabetic Retinal Photography and Nurse Counselling Services
- Assist in implementation and educating patients on Healthier SG and benefits.To work closely with the GPs to manage identified patients who need support and referral to community services and ensure that their changing needs are addressed
- To track, Monitor and follow up on the quality of care provided by GPs and other Primary care professionals and to ensure the maintenance of good clinical care.
- Competent in data entry in Chronic Disease Registry and providing onsite training to Clinic assistants
- Assist GPs in the Submission of data and other relevant administrative work
- Conduct Quarterly audits to ensure data is entered correctly
- Involve in the PCN planning team to ensure smooth coordination within PCN
