Responsibilities:
- Provide home-based psychosocial rehabilitation, follow-up on treatment and support to patients and their caregivers in the community.
- Plan, implement and document effective and comprehensive care plan with the multidisciplinary team for client on identified problems/needs.
- Monitor clients’ progress, supervise their medication and administer injections as prescribed by the doctor.
- Collaborate with the Community Nurses from other PHIs in the management of clients.
- Conduct sharing and training for Community Partners.
Requirements:
- Registered Nurse with SNB
- Possess a Registered Mental Health Nurse /Adv Dip Mental Health or other relevant qualifications (e.g. social work, counseling, psychology)
- Working experience in psychiatric setting (with Adv Dip in Mental Health) with at least 6 months of experience in acute setting, or experience in psychiatric setting for at least 4 years and 6 months of experience in acute setting
- Passion to work in the community
- Possess good interpersonal, communication, writing & IT skills
- Familiar with healthcare environment
- Able to work independently with good decision-making skills
