Job scope

  • Programme planning and implementation for active ageing programme, befriending and buddying, information, and referral to care service, and management of emergency of alert response call.
  • Do Community Screening to identify the needs.
  • Provide and promote a variety of activities such as karaoke, arts & craft, cooking, and exercise programmes.
  • Develop and implement Outreach plan/follow-up visits to seniors in the identified blocks.
  • Provide social support to seniors through house visits and phone calls.
  • Provide information on schemes, grants, and support.
  • Raise referrals to appropriate services.
  • Develop and implement annual calendar plan of social and health activities for seniors.
  • Ensure that the programmes and activities meet the performance indicators and operating guidelines issued by MOH, AIC via the reporting systems, administration of the Client Satisfaction Questionnaire (CSQ), half-yearly report on the programmes and services, outcome management framework, etc. Volunteer Management
  • Establish and motivate seniors to run mutual help groups.
  • Conduct activities and programmes of the AAC to promotes a community climate of multi-racial tolerance, harmony, and co-operation among residents.
  • Network with other agencies to establish additional avenues of help for the welfare of the seniors.
  • Volunteer Management
  • Assist to establish networks with local community-based services/resources, such as grassroots, social services, corporate organisations, and schools, to engage volunteers to help run or assist in the programmes, start income generating activities, conduct outreach efforts, etc to match the needs of the seniors.
  • To Recruit, interview and screen potential volunteers.
  • To Provide and coordinate orientation of volunteers to their assigned role and the mission and objectives of the AAC.
  • To Place volunteers to suitable positions and areas of need, such as administration of CSQ etc.
  • To supervise and support volunteers.
  • To establish a system of recognition for the volunteers.
  • To review and evaluate the volunteer programme to ensure relevance and to meet future needs.
  • To serve as the key communication link between the AAC and its volunteers.
  • Quality Assurance
  • Involve in quality procedures and guidelines and policy reviews when required.
  • Apply compliance to the Company’s quality procedures, guidelines, and policy. Monitor and report non-compliance and implement necessary changes resulting from quality assurance findings in consultation with Centre Manager
  • Staff Development / Education
  • Attend courses relevant to job functions.


Requirements

  • At least O level & above
  • Any other knowledge required
  • At least 2 years’ working experience.
  • Good communication & interpersonal skills
  • Preferably with interest and passion for the elderly
  • Basic Computer skills for activities, documentation and recording.
  • Experience in healthcare/community care setting would be an advantage
