Duties
- Assesses and identifies needs of patients and families on admission to any of the services and during regular reviews.
- Assesses and reviews patients and families in timeframe stipulated.
- Facilitates discussion of patient’s and family’s goals of care.
- Discusses with Head Medical on admission and establishes written plan of care at appropriate intervals.
- Reviews patient’s condition in a timely manner & institute appropriate treatment accordingly.
- Evaluates the benefits and sideeffects of treatment provided.
- Supports nursing and allied health colleagues in their learning.
- Participates in after hours on-call coverage as per rota*.
- Liaises with and hands over of patients’ care to colleagues on duty.
- Responds to on-call telephone calls in an appropriate and timely manner.
- Performs clinical procedures (phlebotomy, intravenous cannulation, setting up a syringe driver, urinary catheterization, wound debridement) where appropriate.
- Documents accurately the assessment of patient and family, goals and plan of care, treatment evaluations and reviews.
- Documents clearly and legibly the medication orders in the medication record.
- Communicates in a timely fashion with colleagues in the interdisciplinary team to ensure holistic patient care.
- Communicates in a timely fashion with other healthcare workers in other institutions to ensure good continuity of care.
- Demonstrates good teamworking and maintains healthy relationships with clinical and non-clinical colleagues.
- Facilitates access to appropriate supportive care services together with the interdisciplinary team.
- Participates in 1-2 weekly interdisciplinary team meetings to review active cases.
- Provides written communication where necessary.
- Provides advice to colleagues in other organizations or directs to other team members as appropriate.
- Issues certification of cause of death according to regulations.
- Provides orientation for new doctors on attachment.
- Participates in educational activities such as Journal Club and tutorials.
- Participates in guiding medical students and other visitors on attachment
- Provides briefing and/ordebriefing for medical students on attachment.
- Manages annual or study leave sensibly to ensure adequate manpower for patient care.
- Assists in application for nursing home or other suitable care facilities for patients as required.
- Participates in Quality Improvement and/or Research activities on an ad hoc basis.
Job requirement
- Basic medical qualification recognized by the Singapore Medical Council
- At least 2 years of medical practice in the hospital or community, of which one year should be in acute medicine with on-call experience.
- Work experience in Internal Medicine, Family Medicine, Oncology, Geriatrics or other related specialties would be advantageous.
