Responsibilities
- Homebased patient care (west /central region) - wound management, stoma care
- Conduct assessments and document per guidelines
- Train and support caregivers in home settings
- Educate on wound healing and demonstrate stoma care to caregivers
- Offer emotional counseling to patients and families
- Develop and update personalised care plans
Requirements
- Nursing Diploma/Degree with valid Singapore Nursing Board registration
- Community Care background a plus
- Excellent interpersonal and communication skills
- Independent and collaborative work capabilities
