The Staff Nurse will provide consultative advice and home care services to patients and family members based on care needs.
He / She will also support families and care givers of patients and work with the rest of the multi-disciplinary team to provide symptom management, nursing care, psychological, emotional and spiritual care.
Responsibilities:
- To uphold nursing standards for patients admitted to homecare as well as care for families and loved ones.
- To work in collaboration with the Advanced Practice Nurse, NC/NM/NE and other nursing staff as appropriate to put in place clinical processes that are conducive to the maintenance and improvement of standards of nursing care in Home Care.
- To ensure accuracy of all documentations related to patient care in the system.
- To ensure holistic assessment of patients in DPH Home Care and their families and caregivers in order to formulate a core plan.
- To be part of the nursing teaching faculty at DPH as required and to be part of the process of quality improvement through review of care processes, active participation and/or leadership in projects as required.
Job Requirements:
- State Registered Nurse with minimum 5 years of nursing experience in acute care setting, and 3 years as a staff nurse in a palliative care setting.
- Post-Basic training in Palliative Care Nursing is preferred.
- Training in lean methodology, audit and research is preferred.
- Ability to speak a second language or other local languages
- Mature, dynamic, and self-motivated.
- Possess excellent communication and interpersonal skills.
- Self-directed with the ability to work with little supervision.
- Comply with accepted professional standards and practices.
- Possess satisfactory references from employers and / or professional peers.
- Flexible and cooperative in fulfilling all obligations.
