Job Description:
- Process and manage referrals for ACP discussions from inpatient, outpatient and community settings of SLH
- Facilitate ACP discussions with patients, families and nominated spokespersons
- Document ACP discussions at the appropriate platforms
- Coordinate between patients, families and healthcare team for ACP facilitation and follow up
- Make referrals to community services related to ACP matters
- Ad hoc duties as assigned
Job Requirements: Min Degree in Social Work with ACP Training and 3 years relevant experience
