Join our dynamic and vibrant department as an MSW in Yishun Campus and make an impact on the lives of patients and their families. We are seeking compassionate and trained individuals to support our acute, community hospital, ICU, A&E, community and outpatient units.
As an MSW, you will be responsible for assessing biopsychosocial-spiritual needs of our patients and working closely with multi-disciplinary team to establish a holistic care plan and carry out appropriate interventions which may include counselling and home visit.
We emphasize the importance of social-health integration. You are required to mobilize hospital and community resources in empowering patients to successfully integrate back home. You will collaborate with community and social agencies and may support in programs to enhance quality of life of patients and their caregivers.
Our campus adopts Kaizen approach and looks for continuous improvement when caring for patients. You will be given opportunity to support in improvement projects/ innovation and learning activities in enhancing the effectiveness and efficiency of our hospital services.
Qualifications:
- Degree or Postgraduate Diploma in Social Work is required
- At least 3 years of social work experience preferably in clinical setting
- Strong advocate for holistic and patient-centred approach to care of patients with medical condition(s) and/or disability
- Must be a team player who enjoys working in a fast-paced environment
- Open to job rotation at different setting ie Acute Inpatient, Community Hospital, Outpatient and Community
- Being a registered member with the Singapore Association of Social Workers
