Job Scope:
- Carry out assessment of patients’ discharge plans and ensuring patient’s readiness for discharge.
- Facilitate referral process by communicating with patients, family members and various internal stakeholders.
- Work closely with multi-disciplinary team to facilitate discharge process and ensuring discharge before noon (DBN) KPI is met.
- Monitor the referral trends and identify service gaps for improvement.
- Document assessment, care plans and outcomes in a timely and accurate manner in EPIC.
- Any other ad-hoc as assigned.
Job Requirements:
- Minimum a Health Science / Nursing Degree
- Good communication and writing skills
- Proficient in MS Office applications
