Job Description:

As a Case Worker, you will be responsible for providing comprehensive support and assistance to community members, particularly seniors, in accessing health and social care services. Your role will involve assessing their needs, coordinating services, and implementing individualized care plans to promote well-being and community engagement.


Key Responsibilities:

Case Management:

  • Assess the health and social care needs of community members.
  • Coordinate support services and placement as required.
  • Develop and pilot workflows, protocols, and processes to integrate social and health care effectively.
  • Collaborate with seniors to develop and implement a calendar plan of social and health activities, fostering a community climate of tolerance, harmony, and cooperation.
  • Utilize a strengths-based practice model to work with service users, including identifying needs, conducting care assessments, formulating individualized care plans, and monitoring progress.
  • Identify and assess the strengths and limitations of community assets, including other Community Care service providers.
  • Facilitate service user participation in programs and activities, networking with relevant community organizations to develop and strengthen services.

Community Engagement:

  • Conduct community profiling to identify needs, interests, and capabilities of residents through methods such as door-knocking, surveys, and interviews.
  • Provide opportunities for service users to remain connected to the community.

Administrative Support:

  • Assist in the supervision and/or training of persons/students on placement.
  • Conduct presentations to external parties as required.
  • Maintain up-to-date statistics and records of services rendered.
  • Liaise with outside agencies regarding service user matters.
  • Participate in staff development programs and productivity efforts.
  • Perform general administrative duties and provide clerical support.
  • Provide guidance, on-the-job training, and SOP sharing to staff or volunteers.
  • Provide secretariat support to staff meetings and other duties as required.

Requirements:

  • Bachelor’s degree in social work, psychology, sociology, or related field.
  • Previous experience in social services or community outreach preferred.
  • Strong interpersonal and communication skills.
  • Ability to work effectively in a diverse community setting.
  • Proficiency in Microsoft Office Suite.
  • Ability to prioritize tasks and manage time effectively.
  • Empathy, compassion, and a commitment to serving others.
