- Coordinate care for client with relevant stakeholder
- Manage cases and follow up duties
Job Requirements
- Min Bachelors Degree in Social Work from a recognized institution
- Min 1-3 years’ work experience in social work, preferably in a social service or healthcare setting would be good
- Min 3 years of relevant working experience & 1 year of holding supervisory position for Senior position application
- Strong interpersonal, communication skills
- Candidates with less experience will be considered
