To provide effective and efficient physiotherapy sessions and develop innovative solutions to meet the client needs, in line with best practice. He / She will work with a multidisciplinary team to ensure the coordination, development and delivery of a quality, client-centered therapy service.
The physiotherapist will also be required to support other departments in providing clinical support where necessary, to ensure a seamless journey for our clients and their caregivers.
Responsibilities
Clinical Duties
- Conduct accurate and holistic physiotherapy specific assessment for clients during admission, periodic care plan reviews and discharge
- Select appropriate and specific outcome measures to enable evaluation of therapy progress and outcomes in a timely manner to guide clinical decision making
- Formulate person centered and SMART goals in collaboration with clients and relevant stakeholders
- Design plans and conduct physiotherapy interventions for clients
- Document assessment results, therapy plans, interventions, goals and outcomes measures according to guidelines
- Communicate assessment results, intervention plans, goals, outcomes and discharge plans to clients and relevant stakeholders
- Recommend appropriate assistive device and equipment as needed
- Formulate discharge and continuity of care plans based on clients’ performance in consultation with relevant stakeholders
- Facilitate community integration through collaboration with other community partners as required
- Empower clients and caregivers with methods to enhance health, mobility and functions through self-management strategies
- Participate in case discussion with the care team using a multidisciplinary, collaborative approach
Requirements
- Degree or Diploma in Physiotherapy
- AHPC Full Registration
- Able to converse in English and other languages
- Minimum 2 years relevant experience preferably with neuro-rehabilitation
- Dynamic and proactive personality with good public relation and communication skills
- Passionate in serving the community
