The Role
We are looking for a dedicated and empathetic Palliative Care Nurse to join our growing team! In this role, you will play a vital part in ensuring our patients receive holistic care, focusing on managing pain and symptoms while providing emotional and spiritual support.
Responsibilities
- Conduct comprehensive assessments of patients' physical, emotional, and spiritual needs.
- Develop and implement individualized care plans in collaboration with the interdisciplinary team (doctors, social workers, chaplains).
- Administer medication to manage pain and other symptoms.
- Provide emotional support and counseling to patients and their families.
- Educate patients and families about the end-of-life process and available resources.
- Collaborate effectively with other healthcare professionals to ensure seamless care coordination.
- Maintain accurate and confidential patient records.
- Participate in interdisciplinary team meetings and case discussions.
Qualifications
- Registered Nurse (RN) with the Singapore Nursing Board (SNB).
- Valid Post-Registration Nursing Qualification in Palliative Care or a minimum of 2 years of experience in a palliative care setting.
- Strong clinical skills and a deep understanding of palliative care principles.
- Excellent communication and interpersonal skills, with a compassionate and empathetic approach.
- Ability to build rapport with patients and families from diverse backgrounds.
- Emotional resilience and ability to cope with challenging situations.
- Strong teamwork and collaboration skills.
- Proficiency in electronic health records (EHR) systems.
