RESPONSIBILITIES
The incumbent will provide service coordination management, administrative support, and operations improvement and analysis for our Day Rehabilitation Centres (DRC).
a) Service Coordination Management
- Manage the onboarding process for new clients, assist in appointment scheduling and re-scheduling, subsidies application, and address client inquiries for Rehabilitation and Physiotherapy services.
- Work closely with internal stakeholders (where applicable) to ensure the smooth delivery of service.
- i.e. Active Rehabilitation (AR) service is carried out by Therapists.
b) Administrative Support
- Plan and remind stakeholders of the reassessment of care plans with clients as per their service plan's requirements. This is to determine if the client needs to continue with the service.
- Ensure that proper documentation on the systems and paper filing is completed on a timely basis.
- Provide scheduling and planning support for Day Rehabilitation Centres and Nursing Homes in accordance with planning guidelines.
- Assist the Rehabilitation and Physiotherapy team with client enrolment and center schedule management, and Bumblebee (CMS system) administrative and support tasks.
- Provide any other ad-hoc administrative general support to the Rehabilitation and Physiotherapy team as assigned.
c) Operational Improvement and Analysis
- Extract, measure, and monitor key performance indicators for Rehabilitation and Physiotherapy services.
- Assist Rehabilitation and Physiotherapy centers in driving center utilization metrics.
- Identify areas for operational improvement to increase the efficiency of Rehabilitation and Physiotherapy centers.
d) Additional Responsibilities
- Assist the Care Coordinator team in service coordination management.
- Diploma or Advanced Diploma, preferably in healthcare related disciplines.
- Minimum 3 years' experience working in a healthcare settings in Singapore
- Good knowledge of the geriatrics and community services
- Proficiency in MS Office applications
- Proficiency in Google Workspace applications -
- Gmail, Google Calendar, Google Docs/Sheets/Slides, Google Drive
- Good interpersonal and writing skills
- Good team player and able to multi-task
- Good communication skills (oral and written)
- Passionate in community services and service-oriented
- Good judgment and decision-making, problem-solving skills
- Independent, resourceful, and self-motivated
- Meticulous with an eye for detail
