Care Coordinator (Home Care)

  • Manage home care client data base to ensure completeness and accuracy.
  • Process home care referrals through AIC referral system as point of liaison.
  • Peform basic financial assessment for clients or next-of-kin on home care service(s) required and refer to social worker if financial assistance is required.
  • Coordinate internal and external referrals of services required (e.g referring home personal care client for home nursing or medica social worker).
  • Coordinate with volunteers to engage home care clients meaningfully.
  • Coordinate and participate in multi-disciplinary discussions to address the social and clinical needs of the clients.
  • Respond to complaints and feedbacks from clients and next-of-kin.
  • Manage and schedule deployment of care staff/clinicians for clients based on their preferred date, time and frequency of service, etc.
  • Check and verify invoices based on the services rendered every month.
  • Collect relevant and accurate data for reporting to management and regulatory authorities.
  • Participate in project work (e.g quality improvement within department/orgnaisation) to improve the quality of care and services to patients.
  • Identify service gaps in response to client's needs and make recommendations to improve service provision of home care.
  • Any other duties assigned by reporting officer.
