Case Management
- Perform needs assessment of community dwelling seniors
- Facilitate timely and cost appropriate service referral and provision to seniors
Work with the SAC team to design, implement activities and wellness programmes that focuses on building the elderly’s self-help capacity including:
- On-site active ageing programmes
- Organising and participation in community events and outings
Administration – assist the Manager
- Design, implement activities and wellness programmes that focuses on building elderly’s self-help capacity including
- Ensure compliance to Finance and Human Resource Policies, maintaining documentation for audits facilitated by Corporate Services
- KPIs of funded programmes
Job Requirements:
- Diploma in Social Work
