As Assistant / Care Coordinator (Primary Care Partnerships), you are responsible for the following areas:
Job Responsibilities
* Perform patient selection, patient counselling, appointment consolidation and referrals
* Provide training and develop workflows for hospital departments to support them in the right-siting efforts
* Support the continuing care of patients in the community
* Facilitate the transition of patients
* Enter data into database for tracking
* Engage patients for health promotion activities and participation in initiatives as and when required
* Perform any duties as assigned by supervisors and HOD
Job Requirements
* Min Diploma / Degree in Health or Social related field
* No experience required as training provided
