RESPONSIBILITIES
Provide client care
- Provide personal care assistance to residents in their daily living such as toileting, bathing, dressing and grooming and ensure the residents good personal hygiene at all times
- Provide supervise or assistance to residents during meal times
- Perform daily activities residents management according to care plans (ICP)
- Prepare residents for their various activities, meal times and bed times
- Assist in giving regular medication and when necessary, other medication under Staff Nurse's supervision with proper recording
- Ensure the proper records of residents as instructed. (e.g. behaviour records, daily logs, home leave records, etc)
- Ensure that residents diet restrictions are adhered to
- Monitor and record bowel movement, menstruation, blood pressure, weight and height of residents on a regular interval
- Conduct body checks on residents on a daily basis
- Support the intervention during medical emergency of residents
- Maintain up-to-date documentation of clients' conditions and care treatments
- Provide inputs during case review and liaise with healthcare professionals for the development and review of care plans
- Verify accuracy of documentation of client's condition, progress and care treatments
- Review inventory for replenishing in medical supply
Maintain order, safety and cleanliness of the care environment
- Ensure client behaviours are in compliance to house rules
- Respond to unusual or challenging behaviours of clients
- Recommend measures to mitigate risk to clients
- Support the intervention implementation for managing the challenging behaviours
- Perform housekeeping duties
- Ensure equipment is maintained in a clean and working condition
Manage care operations
- Perform daily handover to staff during changes in shifts
- Provide input to operations review meetings
Perform stakeholder engagement
- Assist in liaising with other professionals, volunteer Doctor, hospital and stakeholders involved in the case to facilitate provision of care services for clients
- SQ Higher Certificate in Healthcare (Nursing Care)
- ITE Skills Certificate in Healthcare (Home Care) Programme
- Preferably 1-year relevant working experience
- Computer literacy skills MS Word and PowerPoint
- Experience working with persons with autism, intellectual and/or developmental disabilities is preferred
- Able to commit 3 working shifts / 5 days work week
