- Engage and collaborate with active ageing programme partners and internal AIC divisions to support frailty management for robust and frail seniors (emphasis: prevent/manage frailty) in alignment with National Frailty Strategy Policy guidance and other national directives.
- Research, plan and implement evidence- and interest-informed initiatives at the Active Ageing Centres (AACs) for robust and frail seniors in collaboration with partners and the Community Care sector
- Review activity impact to explore further enhancements to better meet activity needs at the AACs for both robust and frail seniors
- Analyse information, data and ground insights to identify trends, needs and gaps and formulate strategies to support activity provision at the AACs
- Forge and develop new strategic and sustainable partnerships to advance ecosystem of activities for both robust and frail seniors
- Develop strategies and plans, in collaboration with internal and external stakeholders, on differentiated engagement approaches to better reach out to seniors, especially under-represented segments, to encourage activity awareness and participation at the AACs
- Review enablers to support activity development and delivery with key active ageing programme partners (e.g. capability building of staff, volunteers, partner agencies)
- Project manage and evaluate the outcomes and effectiveness of the initiatives
- Engage and manage internal and external stakeholders to gain buy-in and partnerships
Job Requirements
- Degree in any discipline with at least 5 years of relevant experience
- Strong collaborative and partnership building skills
- Strong in programme management and evaluation experience, preferably in healthcare, eldercare and/or social services
- Self-motivated, resourceful with good problem-solving skills and a high level of initiative
- Able to manage multiple projects with excellent project management skills and to thrive under pressure in a fast-paced environment
- Effective communicator (verbal, in writing) with good interpersonal skills and an interest to enhance care for seniors
- Responsible, independent and a good team player, who is able to mentor and guide more junior colleagues
