Responsibilities and Duties
· Perform social and financial assessment of referrals including financial counselling to patients and their families.
· Provide the necessary recommendations in the assessments.
· Prepare monthly statistical analysis, reports and submissions to key stakeholders in a timely manner.
· Assist in developing and implementing quality improvement initiatives to streamline department operations, to enhance patient outcomes and satisfaction.
Job Requirements:
· Degree in Social Work or equivalent, preferably with 3 years’ social work experience.
· Must have an interest to work with the elderly in a hospital environment.
· Can work independently and under pressure.
· High level of competence with statistical analysis tools and proficient in Microsoft Office
applications, in particular, Microsoft Excel.
