· Ensure the pharmacy work area is neat and clean
· Develop and regularly review of Job Instructions and Job Improvements for department work processes as well as training and equipping staff to carry out their work correctly, safely and conscientiously
· Promote a working environment of collegiality and elective teamwork
· Any other duties as assigned by the Supervisor / Head of Department
· Maintain consistent standards of care and ensure that patients receive their medications in an accurate and timely manner
· Promote rational drug therapy selection and recommend appropriate, cost-elective therapeutic alternatives to prescriber where necessary
· Dispense and counsel patient on their medication according to operational and legal requirements
· Perform medication reconciliation to ensure accuracy of medication orders against the past history and to gauge medication compliance
· Review medication orders to determine the pharmacotherapy needs of the patient and ensure that these needs are met through recommendations to doctors.
· Check medication order for contraindications, drug-drug Interactions, drug-food interactions, drug-laboratory test interactions, drug-disease interactions, allergies, appropriateness of drug, route and dosage, and therapeutic duplications
· Provides retail sales according to operational and legal requirements
· Provide accurate, adequate, and timely drug information to the professional staff as well as patients and caregivers
· Actively participate in clinical or quality improvement projects at Departmental level
· Inform prescriber of any prescribing error, significant drug interaction, adverse drug reactions or contraindication.
· Detect and report suspected adverse drug reactions and medication errors in an accurate and timely manner to help avoid future incidents
