Responsibilities
Specific
- Manage and coordinate referrals for Advance Care Planning (ACP) discussions from various healthcare settings, including inpatient, outpatient, and community environments.
- Facilitate ACP discussions with patients, families, and nominated spokespersons to ensure informed decision-making.
- Accurately document ACP discussions using appropriate platforms and protocols.
- Serve as a liaison between patients, families, and the healthcare team to facilitate ACP processes and ensure follow-up.
- Refer patients to community services related to ACP matters to enhance comprehensive care.
General
- Collaborate with the ACP Coordinator to support ACP referrals, enrolment, and data management.
- Conduct educational talks and workshops for healthcare professionals to promote awareness and advocate for Advance Care Planning.
- Participate in public awareness events, both within the institution and in collaboration with external partners, to raise community awareness about Advance Care Planning.
- Assist in conducting audits, research, surveys, or quality improvement projects focused on Advance Care Planning.
- Prepare reports and statistical data as required to monitor and evaluate ACP initiatives.
Requirements
- Possess a professional qualification in Nursing, Social Work, Allied Health, or a related clinical discipline.
- Minimum of 3 years of experience as an Allied Health Professional, preferably in a hospital setting.
- Certification in Advance Care Planning Facilitation (General and Preferred Plan of Care) is highly desirable, along with proven experience in conducting ACP discussions.
