The Community Health Visitor (CHV) will deliver integrated preventive health and social care service provision and coordination for mother and child, focusing on antenatal and postnatal care, developmental-centered parenting, parent child interaction and family well-being, with the aim of optimising every child's potential for healthy growth and development. To be the main point-of-service-coordination between patient/client/family with healthcare and social service systems.
Job requirements:
- Degree in Nursing (preferred), Early Childhood, Psychology or
- Specialist Diploma (Early Childhood/Psychology/Social Work) with 5 years relevant experience
- Relevant Experience of working with vulnerable children and families in a community setting and/or with infant and child development work
- Scope of work is physically and emotionally demanding
- Includes travelling to and from Client's homes for service delivery and emotionally intense work with vulnerable families, children and young infant
- Dynamic, problem-solver, efficient, resourceful and independent individual
- Demonstrate a high level of initiative and organisational skills
- Strong interpersonal, communication and writing skills are essential
