As Assistant / Care Coordinator (Primary Care Partnerships), you are responsible for the following areas:


Job Responsibilities

  • Perform patient selection, patient counselling, appointment consolidation and referrals
  • Provide training and develop workflows for hospital departments to support them in the right-siting efforts
  • Support the continuing care of patients in the community
  • Facilitate the transition of patients
  • Enter data into database for tracking
  • Engage patients for health promotion activities and participation in initiatives as and when required
  • Perform any duties as assigned by supervisors and HOD


Job Requirements

  • Min Diploma / Degree in Health or Social, psychology related field
  • No experience required as training provided
