As Assistant / Care Coordinator (Primary Care Partnerships), you are responsible for the following areas:
Job Responsibilities
- Perform patient selection, patient counselling, appointment consolidation and referrals
- Provide training and develop workflows for hospital departments to support them in the right-siting efforts
- Support the continuing care of patients in the community
- Facilitate the transition of patients
- Enter data into database for tracking
- Engage patients for health promotion activities and participation in initiatives as and when required
- Perform any duties as assigned by supervisors and HOD
Job Requirements
- Min Diploma / Degree in Health or Social, psychology related field
- No experience required as training provided
