RESPONSIBILITIES
Our Medical Social Workers collaborate with a multi-disciplinary team of stakeholders to regularly review individual residents' medical, nursing, therapy, and social aspects as part of delivering person-centric care. They will be involved in the means-testing review, and provide fair payment proposals, financial application assistance, and any appropriate assistance based on the social & economic status of the family and/or resident.
Admissions
- Provide financial counseling upon admission
- Co-ordinate an assessment on site of the patient to assess nursing/rehab status/behavioral and any social concerns
Case Conference
- Review social aspects inward with regards to activity participation, visitors and befriending, financial consideration status, etc.
- Provide therapy/nursing/medical/social recommendations and support family and resident to complete home discharge or other care options
- Review supporting documents for referral to other care options and seek financial assistance for home equipping as recommended by the therapist where required
- Provide counseling to patients with psychosocial issues
- Facilitate family conferences for discussion on financial considerations, if necessary
- Work with the Nursing team to review care for residents and understand their social situation
Financial Assistance Schemes
- Work with resident or next-of-kin on financial considerations (e.g. Government Means Test subsidy)
- Review cases and facilitate meetings for Medifund cases
- Apply for financial assistance (e.g. Means Test, Medifund, IDAPE, ELDERSHIELD, MFEC)
Reports
- Update residents' profile and movement in database
- Review next-of-kin details and update residents' social report
- Review residents accounts receivable status report
Supervision/Development of team members
- Manage and supervise team members in case management team
- Guide on the professional development of team members
- Degree in Social Work or related discipline
- At least 5 years of relevant experience
- Social work in healthcare background preferred
- Good interpersonal and problem-solving skills
- Caring, positive and self-disciplined individual
- Ability to work well in a team
- Proficient in Google Workspace (i.e. Gmail, Google Docs, Sheets, Slides etc)
