Job Scope:
- Conduct community outreach and relevant need assessments to the elderly at a designated service boundary.
- Conduct outreach services to frail and vulnerable elderly at their homes.
- Provide referral to help services for the elderly in needs.
- Recruit volunteers to assist with the outreach and services.
- Train volunteer to provide monitoring and appropriate referral to support services.
- Perform administrative and reporting duties such as visitation notes, records and other necessary data and statistics required by the authorities.
- Perform other duties as assigned by the reporting officer.
Job Requirements:
- Minimum Diploma / Degree from a recognized institution
