Role Description:


The Allied Health Professional or equivalent will be responsible for organizing and conducting outreach to increase mental health and dementia awareness and promote early identification. He/she will support clients with or who are at risk of mental health issues and/or dementia by linking them to the necessary support and resources.


He/She will continue to monitor and support these clients in the community.


The Allied Health Professional will also be involved in data collation and evaluation of the CREST programme.


Responsibilities/Key Tasks:

A. Programme Development and Implementation

i. Provide care coordination and service linkage to clients with or who are at risk of mental health issues and/or dementia, with the appropriate resources and support including caregiver support.


ii. Plan and conduct outreach to the community and stakeholders including faith-based, corporate and community organisations to promote mental wellness and mental health awareness.


iii. Plan and conduct targeted mental health and dementia case finding/screening to promote early identification and facilitate timely support.


iv. Plan and organize structured preventive activities and/or programmes for the community, those at risk of mental health conditions and clients.


B. Administration and Staff Development

i. Responsible for maintaining client records that are accurate and timely for submission to AIC for data analysis, in accordance to the CREST programme requirement.


ii. Keep and maintain client and programme data and documents, in accordance to the CREST programme requirement, as well as the prevailing personal data protection and security guidelines.


iii. Support in the development of CREST service planning document and manual including set up requirements (infrastructure, manpower, training, recruitment) and processes (outreach, screening, mental health information, service linkage & follow-up).


iv. Ensure attendance in training and supervision sessions, where relevant, as part of the staff competency required by the CREST programme.


C. Other Duties

i. Participate in relevant meetings with key stakeholders including AIC to share key learning points where required.


Qualifications:

· Degree or Post Graduate Diploma in Social Work or related disciplines from recognised universities;

· At least 3 to 5 years of relevant experience. Preferably a registered social worker
