Responsibilities
- Review medication orders and perform and document interventions if required
- Process drug orders and dispense medications to patients
- Counsel patients on their medications and provide specialised counselling if needed
- Perform medication reconciliation
- Provide drug information to doctors, nurses and patients
- Assist in the preparation of ad-hoc items
- Other duties as assigned
Qualifications & Experience
- Bachelor of Science in Pharmacy
- Registered with Singapore Pharmacy Council
