RESPONSIBILITIES
- Assess referrals and co-ordinate to process placement applications.
- Responsible for all admission and discharge procedures of residents.
- Maintain records on the progress of the resident/referral
- Conduct means testing, social and financial assessments of residents and their families.
- Provide financial counselling to residents and their families, assist with financial applications where necessary.
- You will provide psychosocial support, counselling and practical assistance to our residents and their families
- Work with the multi-disciplinary team on holistic care management of residents.
- Prepare weekly bed vacancy report and monthly residents’ list
- Engage families’ involvement and encourage their visit.
REQUIREMENTS
- Degree in Social Work or Graduate Diploma in Social Work
- Accredited as “Registered Social Worker” with SASW
- At least 3 years of experience in medical social work preferable in a Nursing Home
- Familiar with MS office tools
- Able to write well in English and have good problem solving skills.
- Able to converse with residents in Chinese dialects
- Team player and self-discipline
