The Social Work Associate is responsible to supporting the aftercare casework service, data tracking, alumni program and study grant administration.

Key Responsibilities:
Aftercare Services
· Provide comprehensive assistance in case management services, including:
- supporting clients in their transition to independent living.
- implementing personalized transition plans.
- addressing potential barriers to a successful transition.
- following up on case plans and interventions.
- Data Tracking and Analysis
· Maintain accurate and up-to-date records and databases, ensuring data integrity and confidentiality.
· Track client progress and outcomes, monitoring individual success and identifying areas for program improvement.
· Conduct data analysis to identify trends, patterns, and areas for improvement, informing program development and enhancement.
Alumni Program and Engagement
· Assist in organizing and coordinating alumni events, fostering a sense of community and connection among program graduates (twice a year).
· Engage with alumni to provide ongoing support, guidance, and resources, promoting continued growth and success.
Study Grant Administration
· Assist in managing study grant applications and disbursements, ensuring compliance with grant requirements and regulations.
· Coordinate with educational institutions and funding agencies to facilitate grant processing and disbursement.
Other Duties
· Provide additional support in case management and client support as needed, ensuring seamless service delivery.
· Participate in team meetings and training sessions, contributing to program development and staff growth.
· Perform other duties assigned by the Supervisor
Requirements:
· Diploma or equivalent in Business Administration, Social Work or related social sciences
· Minimum 1 year of relevant experience in children, youth, or family
· Organizational and time management skills
· Ability to maintain confidentiality and professional boundaries
· Flexibility and adaptability in a fast-paced environment
· Commitment to ongoing learning and professional development
· Bilingual proficiency (English and Mother Tongue/Dialects)
· Ability to work independently and as part of a team
· Resilience and ability to manage stress
