Job Overview:
- Intake and Assessment:
- Assessment of home environment risks through home visits.
- Counselling: To provide counselling and emotional support to patients and family members where needed to help with their coping
- Casework: To work with the team on case management of cases via huddles and multidisciplinary case conference.
- Case direction and assessment of risks for cases.
- Facilitation of weekly huddles to ensure accountability and have an overview of all cases.
- Facilitation and advocacy of needs through the monthly multi-disciplinary meetings to ensure holistic management and communication of higher risk cases.
- Interagency case conference to communicate and advocate for patients' needs.
- Direct support for crisis cases or high-risk cases by providing priority response to families in personal crisis including attempted suicide or have vulnerable children victims of abuse.
