- Intake and Assessment: To conduct FAST (Family and Adult Tool Assessment) and ACE, (Adverse Childhood Experiences) via a home visit upon enrolment of a pregnant mother to the program. Assessment of home environment risks through home visits.
- Counselling: To provide counselling and emotional support to patients and family members where needed to help with their coping
- Casework: To work with the team on case management of cases via huddles and multidisciplinary case conference. Case direction and assessment of risks for cases. Facilitation of weekly huddles to ensure accountability and have an overview of all cases. Facilitation and advocacy of needs through the monthly multi-disciplinary meetings to ensure holistic management and communication of higher risk cases. Interagency case conference to communicate and advocate for patients' needs.
- Direct support for crisis cases or high-risk cases by providing priority response to families in personal crisis including attempted suicide or have vulnerable children victims of abuse.
