Our Client is a non-profit organization, sponsored under the Ministry of Social and Family Development.
They are currently looking for a Social Worker / Case Worker.
Responsibilities:
- Provide Casework and Counselling to teenage girls in a Residential setting
- Liaise, collaborate with, and provide information to external partners and stakeholders
- Coordinate and manage all external appointments of the residents
- Assess risks and needs of residents and their families and formulate the Individual Care Plan (ICP) for each resident accordingly
- Ensure execution of ICP’s planned action according to goals, in close collaboration with care staff and programs team
- Provide counselling to resident’s family, and conduct restorative work between residents and their families
- Monitor daily incidents and progress of residents
- Provide inputs at weekly Disciplinary Committee conferences and ensure execution of outcomes derived from the conferences
- Ensure and maintain accurate and updated records on each resident
- Prepare, review and submit all regular and ad-hoc reports required
- Conduct group work with residents and/or with their families to equip them with the necessary life skills
- Review relevance of all external proposed classes/workshops/talks that may be useful for residents
- Coordinate with the respective agencies on the proposed sessions, internal team and Programs department to facilitate sessions
- Undertake any other duties as and when specified by the Head of Home
Requirements:
- Minimum Diploma in Social Work, Psychology, Counselling, or a related field
- Minimum of 2 years of experience in casework and counselling, preferably with teenages in a residential setting.
- Strong understanding of risk assessment and individual care planning
- Proficient in liaising and collaborating with external partners and stakeholders
- Excellent coordination and organizational skills to manage external appointments and internal activities
- Ability to conduct effective counselling sessions and restorative work with residents and their families
- Proficient in monitoring and reporting on residents' progress and daily incidents
- Strong written and verbal communication skills for preparing reports and maintaining accurate records
- Compassionate and empathetic towards teenages and their unique challenges
- High level of integrity and professionalism in handling sensitive and confidential information.
- Willingness to undertake any other duties as specified by the Head of Home
