Job Description:
- Provide structured nurse counselling, health screening service, patient education on mental health management, chronic disease and health promotion.
- Understand patients' chronic conditions and improve their health outcome.
- Advise patients on how to administer insulin injections, perform self- monitoring of glucose and home blood pressure readings, proper usage of inhalers, provision of targeted diet plans.
- Conduct eye and foot screening with patient education to diabetic patients.
- Work closely with the Primary Care Coordinator on scheduling patients for ancillary services.
- Perform ad-hoc duties as per assigned
Requirements:
- Advanced Diploma in Nursing/ Bachelor's Degree in Nursing
- Registered Nurse with the Singapore Nursing Board
- Specialist Diploma in Diabetes Education/Gerontology or Advanced Diploma in Chronic Disease Management is preferred
